Participation in this and all aspects of the PAR Program is eligible for:

1. Mainpro(r)-C credits with the College of Family Physicians of Canada
   (Members must complete a specific credit submission form to document their process, which can be downloaded from the CFPC website at: http://www.cfpc.ca/local/files/CME/Mainpro/provincial.pdf.)
   
2. Section 5 credits in the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canada
   (Members must document the date of completion, summary of the findings and identified outcomes for practice in accordance with RCPSC requirements.)
