Other versions of the report follow the same format, however, the results pages reflect the questionnaires unique to each physician group. See the Survey Instruments page for the instruments for each physician group.

The physician report is sent to each physician after survey instruments are returned and data compiled from medical colleagues, co-workers, patients and the physician's self-assessment. The physician's results are compared to his or her own practice group.
